PATIENT INFORMATION

Laboratory, Radiology and Hospital Charges

Important information about separate charges from outside healthcare providers, laboratory testing, insurance coverage, and billing responsibilities when receiving foot and ankle care.

UNDERSTANDING MEDICAL BILLING

Why You May Receive Separate Medical Bills

Some diagnostic services involve healthcare providers or facilities that operate independently of Lakeland Foot & Ankle. These providers may bill separately for their services.

Reference Laboratory Testing

Due to certain insurance requirements or the nature of specific laboratory studies, tests may be sent from our office to an outside reference laboratory for processing.

Independent Provider Billing

Reference laboratories are independent of our practice and bill separately for the services they provide. A laboratory charge may therefore appear on a bill separate from your office visit.

Insurance Coverage Verification

Patients are responsible for determining whether the laboratory, radiology or hospital providers involved in their care are covered under their individual insurance plan.

Insurance Claims

Most outside laboratory, radiology, and hospital providers bill patients' insurance companies directly, although separate patient charges may still apply.

INSURANCE AND BILLING GUIDANCE

What Patients Should Know

Before using an outside testing facility or receiving care from an independent provider, understanding insurance participation can help reduce unexpected bills.

01

Identify the Outside Provider

If your care involves an outside reference laboratory, radiology provider or hospital, ask which organization may be providing and billing for those services.

02

Check Your Insurance Coverage

Contact your insurance company to determine whether the outside provider participates in your individual plan and whether the services are covered.

03

Review Any Separate Bill

If you receive a bill from an independent laboratory, radiology facility or hospital, review it with that provider or your insurance company.

04

Contact Our Billing Staff if Needed

If you cannot resolve a billing question directly with your insurance company or the outside provider, please speak with a member of our billing staff.

IMPORTANT DISTINCTIONS

Office Charges vs. Outside Provider Charges

Knowing who provides a medical service can help explain why billing statements may come from more than one organization.

Lakeland Foot & Ankle

Charges from our practice relate to services billed by Lakeland Foot & Ankle. Our Insurance and Office Payment page explains the practice's general payment policies.

Independent Healthcare Providers

Independent reference laboratories and other outside healthcare facilities may bill separately. Patients should verify insurance coverage for these providers under their own plans.
